Meckel’s diverticulumis one of the commonest congenital malformation in gastrointestinal tract. Perforated meckel’s diverticulitis is the rarest complication of Meckel’s diverticulum. Most of the time it mimics with perforated appendix on presentation and is an incidental finding. Our patient is a 26 years old male presented with clinical features of generalized peritonitis. He was taken to the operation room with suspicions of perforated appendix, but interaoperatively it turned out to be perforated Meckel’s diverticulitis which is a rare differential diagnosis of diffuse peritonitis as perforated peptic ulcer disease, appendix and bowel due to trauma are common cause of diffuse peritonitis. The diagnosis difficulty of perforated Meckel’s diverticulum is discussed.
